The Hospice Team Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Team Coordinator plays a crucial role in coordinating and overseeing the delivery of hospice care services to patients. S/he maintains accurate records and facilitates communication and collaboration among team members, ensuring that patients receive the highest level of care, comfort, and support.Position Specific Responsibilities include accurately entering and managing patient data, ensuring admission documentation completeness, using HCHB system for record-keeping and reporting, monitoring durable medical equipment and medical supplies orders, verifying and uploading visit records into EMR and HCHB, ensuring accurate billing, collecting decertification paperwork, initiating medical records requests, facilitating interdisciplinary team coordination, acting as a central contact for hospice team and families, managing phone systems, assisting Hospice Director of Clinical Services with IDT activities, preparing patient binders, and performing other duties as assigned.Education and Experience: High school diploma or GED required; at least two years of experience with medical records, medical office, or business office support highly preferred.Skills required include mathematical skills, language skills for reading and writing reports and correspondence, effective communication, and high integrity including confidentiality of patient records.Physical demands include standing, walking, manual dexterity, lifting up to 25 pounds, and visual acuity requirements.
